ABSTRACT

OBJECTIVE To summariz e the expe rience of the ability training of prescription-auditing pharmacists in prescription pre-audit,and introduce the typical cases of the prescription-auditing pharmacists participating in the drug intervention. METHODS From October ,2020 to October ,2021,under the audit mode of “prescription pre-audit system+prescription-auditing pharmacists ” adopted by Yuxi People ’s Hospital (hereinafter referred to as “the hospital ”),the abilities of prescription-auditing pharmacists were cultivated from the aspects of training in pharmaceutical related professional knowledge ,training in the use of Chinese and English medical retrieval tools ,databases and websites ,and clinical thinking and communication ability ;through the construction of ability evaluation form of prescription-auditing pharmacists ,their abilities were assessed. RESULTS & CONCLUSIONS After one year ’s ability training ,the rational rate of prescription (doctor’s order ),the proportion of doctors ’active revision of problem prescription (doctor’s order )and the doctor ’s acceptance rate of intervention by prescription-auditing pharmacists showed an upward trend ,the average time of irrational prescription (doctor’s order )by prescription-auditing pharmacists showed a shortening trend ,and the intervention rate of prescription (doctor’s order )showed a downward trend. In addition to the publication of papers (belonging to the bonus item ),the average score of the ability evaluation form of prescription-auditing pharmacists had significantly increased , from 45.2 in October 2020 to 97.6 in October 2021.

ABSTRACT

OBJECTIVE:To explore the role of pharmacists i n developi ng the outpatient and emergency prescription pre-audit with the help of rational drug use software. METHODS :With the help of rational drug use software ,outpatient and emergency prescription pre-aduit was conducted during Jan.-Apr. 2020;the intervention effects of irrational prescriptions were analyzed. The prescriptions from Sept. to Dec. in 2019 were included in the control group (only for post review ),and the prescriptions from Jan. to Apr. in 2020 were included in the intervention group (pre-audit and post review );the unreasonable prescriptions were compared before and after the intervention. The prescriptions intercepted by the software system from Nov. to Dec. in 2019 were further selected as the interception control group ,and the prescriptions intercepted from Jan. to Feb. in 2020 were selected as the interception intervention group ;the interception situation of prescriptions before and after the intervention was compared ,and the influence of pre-audit on physicians ’prescription behavior was evaluated. In addition ,108 992 prescriptions in Apr. 2020 were selected for post review using the traditional model (i.e. complete manual audit )and the information model (i.e. software-assisted manual audit )respectively;the effects of two models were compared. RESULTS :From Jan. to Apr. in 2020,a total of 2 393 prescriptions were pre-audited by pharmacists ,among which 1 387 prescriptions were reasonable (57.96%)and 1 006 prescriptions were unreasonable and intervened (42.04%). After pharmacist intervention ,983 prescriptions were modified by doctors ,and the success rate of intervention was 97.71% . Compared with control group ,unreasonable rate of outpatient and emergency prescriptions,that of each type of prescriptions were all decreased significantly (P<0.001). Compared with interception control group(1 402 pieces),the number of intercepted prescriptions (721 pieces)was significantly reduced in interception intervention group (P<0.001). Compared with traditional model , the number of prescriptions requiring manual post review and E-mail:xss306@126.com “false positive ” prescriptions were decreased significantly # under information model , and the number of discovered unreasonable prescriptions were increased significantly (P< 0.001). CONCLUSIONS :With the help of rational drug use software ,prescription pre-audit by pharmacists can significantly improve reasonable rate of outpatient and emergency prescriptions. The work intensity of pharmacists in post review of prescriptions can be reduced and the accuracy of prescription review can be improved in the information mode.

ABSTRACT

OBJECTIVE:To provide reference for improving the rationality of discharged prescriptions. METHODS:According to the prescriptions that were often judged as forbidden or not recommended (such as irrational dosage and course of treatment , off-label drug use ,drug interaction ,medication suitability ,dosage form ) in the pre-audit system of discharged prescriptions (hereinafter referred to as the “audit system ”)of Union Hospital Affiliated to Fujian Medical University (hereinafter referred to as the“our hospital ”),combined with actual case analysis ,the clinical pharmacists looked for evidence-based evidence and evaluate it,communicated with the clinician for feedback ,maintained the content of misjudgment in the intercepted prescription in real time,set up the relevant rules of audit in the audit system ,evaluated the effect and put forward suggestions to improve the scientificity and accuracy of the audit results of the discharged prescription. RESULTS & CONCLUSIONS :For prescriptions that were judged to be forbidden or not recommended by the system ,such as the limited daily dose of Morphine hydrochloride tablet in patients with cancer pain ,the daily maximum dose of calcium sulfate D 3 after thyoidetomy ,hyper-indication use of megestrol acetate after chemotherapy for rectal cancer ,off-label usage of vitamin B 6 after postpartum delivery ,the combined use of clopidogrel bisulfate and oxcarbazepine after intracranial infection ,clinical contraindication of enteral nutrition powder after intestinal obstruction ,unsuitable dose of Dabigatran ester capsule after atrial fibrillation surgery and so on ,the clinical pharmacist looked for evidence-based evidence ,evaluated and communicated with the clinician . The relevant rules were updated to the audit system in real time. On the whole ,the audit system improves the work efficiency and the scientificity and accuracy of the audit results,but it is necessary to improve the professional knowledge level of the pharmacists ,the ability to find and use evidence-based evidence ,the accuracy of the audit system and the acceptance and recognition of the audit results by doctors ,so as to enhance the rationality of the discharged prescription ,reduce medication security risks.

ABSTRACT

OBJECTIVE: To perfect China's pediatric knowledge database of rational medication use, to reduce the incidence of medication errors and to improve the safety and effectiveness of medication therapy in children. METHODS: Construct the pediatric prescription knowledge database by accessing relevant handbooks, guidelines, online databases and researches, and embed it into the prescription pre-audit intelligent decision system. Based on this, the system could automatically recommend dosage and usage information to the physicians during prescribing, it could also pre-screen and intercept improper medical orders and prescriptions according to different security levels. RESULTS: From November 2018 to April 2019, the prescription Pre-audit Intelligent Decision System reviewed a total of 906,945 inpatients orders. Due to the use of the Pediatric Prescription Knowledge Database, the inpatients' precaution, relative contraindication and absolute contraindications orders presented a remarkably decline, from 71.71%, 5.45% and 0.12% respectively, to 35.86%, 3.51% and 0.08%. The physician's fitness to the system and rationality of medication increased month by month. CONCLUSION: The use of the Pediatric Prescription Knowledge Database significantly reduces the incidence of medication errors in hospitalized children and improves the level of medical homogenization. It is a medical service model worthy of consideration.
